I'm writing in regard to making an insurance claim under the terms of my health insurance policy with your company. My policy number is 4837234-09.

I was admitted to Martin Hospital three days ago for a suspected stroke. My wife spoke to one of your representatives by phone at the time of my admission. I was released from the hospital yesterday and am feeling much better. What I would like to know is which documents I need to send to you when I file a claim for my medical expenses. Also, could you please tell me if I will be able to claim for the medicine that the doctor has prescribed for me.
